Guidelines Before Visiting
When you book your first appointment, our EMR (Electronic Medical Record) system will forward you intake and consent form. Please read them carefully and fill them in to the best of your knowledge. This will greatly facilitate your appointment and make it more focused on your treatment, although during the first appointment we still need more time to ask questions and become familiar with your case before proceeding to the treatment .
Policy on Appointment Changes
Our clinic has 24hr cancelation policy, if you need to cancel or reschedule your your appointment please contact us in advance to avoid penalty and inconvenience.

Billing Regulations
Payments are processed by receptionist before or after your appointment, please be sure to pay with credit or debit card. Cash is not accepted as a payment for your visit and treatment.
